Our Proactive Anticipatory Care team supports people with rising health and social risks.

We work in partnership with Richmond GP Alliance to deliver Proactive Anticipatory Care (PAC) across the Richmond Borough.

Our Care Coordinators focus on what matters to you and together explore opportunities in the local community that can help you feel better. They can provide you and your family with support to improve resilience and ensure that your care and support needs are being met.

They work closely with GPs and other healthcare professionals to plan and organise proactive and preventative support around you. This approach allows professionals to work in collaboration towards your goal.

We work with anyone over the age of 18 registered with a GP in Richmond.
