Do you want to take control of your health and wellbeing?

We work in partnership with the NHS and GPs in Richmond to deliver Health in Your Hands.

This project empowers you to take control of your physical health, mental health and wellbeing. Our aim is to bring support into the community and ensure there are no barriers to accessing health and social care, as we know that health conditions are often impacted by social issues.

Our Wellbeing Coordinators can meet with you and find out more about what you want to achieve or change and raise awareness of community services and organisations that can support you.

They will focus on what matters to you and together you’ll explore opportunities in the local community that can help you feel better.

We can support you to:

  • Identify what matters and what is important to you
  • Explore the factors that influence your mental and physical health
  • Access information, advice and guidance to manage your health condition
  • Understand your health needs through a community health check
  • Access a variety of community events that are free to attend

“A lot gets missed in the community and we unfortunately as GPs don’t have the time for detailed visits, especially when those with inequalities don’t feel able to access the service or know what services are available. Social Prescribing and the Health in Your Hands project are helping to reach out to that cohort and provide a holistic approach into physical, mental and social needs”
