Do you want to take control of your health and wellbeing?
Our Community Health and Wellbeing Workers can help!
We are working in partnership with the NHS and local GPs in Hampton North to offer personalised support that helps you access the right local services.
Our Community Health and Wellbeing Worker can visit you at home or meet you in the community. Together, we’ll explore what kind of support you’d like and connect you with services that put you in control and feeling healthier. We can also talk about preventing and/or managing long-term health conditions as a key part of the programme.
We focus on all aspects of life that affect your health and wellbeing, such as:
- Housing issues
- Financial pressures
- Isolation and loneliness
- Employment and volunteering
Fulmer Close residents are welcome to drop in to see us at the Community Hub every Wednesday between 10am to 1pm. Appointments can also be arranged outside of these times if needed.

Looking for employment or training opportunities?
We’re also working with the South London Partnership to help you move into suitable employment, training or volunteering opportunities.
This new programme provides personalised support to help more people overcome barriers and find meaningful work. Whether you’re actively looking for a job or need help to stay in your current role, we can support you to take the next step.
Support includes:
- One-to-one help from an employment specialist
- Creating a tailored action plan and job-ready CV
- Mock interviews and confidence building
- Support with travel and in-work adjustments
- Ongoing in-work support
The programme is free and open to anyone living in South West London. You can also self-refer directly here.
