What is it?
This is a sample of the current PIP form from the government website. You can’t print this off – if you want to make a PIP application you need to contact the PIP helpline and request that a form be sent to you.
Why might it be of interest?
The PIP application form is very different from the DLA application form so it might be helpful to see the form and the questions. Read this in conjunction with the R-PIP Points Table document and/or the R-PIP Guide from B&W (Benefits and Works) to get a good feel for what you will need to do to complete the application for your young person.
