Sample PIP Form

Updated: 22/01/25

Created: 28/11/2023, Bright Futures @Ruils

Who by? Government

Source: View/download article

Why might it be of interest?

The PIP application form is very different from the DLA application form so it might be helpful to see the form and the questions.  Read this in conjunction with the R-PIP Points Table document and/or the R-PIP Guide from B&W (Benefits and Works) to get a good feel for what you will need to do to complete the application for your young person.

The government website has a PIP information page and handbook which you might find helpful:

https://www.gov.uk/pip

https://www.gov.uk/government/publications/personal-independence-payment-fact-sheets/pip-handbook

When you phone the PIP helpline to request a PIP claim form it is likely that you will be asked the questions contained in the PIP1 form over the phone so do be ready for this.  The form will then be sent to you to verify the information.

Three people going down the street talking with each other. One is wheeling herself in a wheelchair and the other two are walking. One has a Ruils tote bag on her arm.
