Why might it be of interest?
Deciding to go for a deputyship for your young person – for either or both property and financial affairs or personal welfare – is a big decision. It might be helpful to see the forms in advance of making the decision – there is a lot of information you need to provide – and reading through might help you decide whether you need to make an application or not.

COP1 – Application to become a deputy
This form must be completed for either type of deputyship.
COP1A – Property and Affairs
Additional information about the types of decisions you need to make. You will need to provide information about their income and assets.
COP1B – Personal Welfare
Supporting information for a Personal Welfare application. To be submitted with the COP1 form.
COP3 – Assessment of Capacity

You can only have a deputyship for a person who lacks capacity to make their own decisions. This form must be completed by someone able to assess the person’s ability to make their own decisions. This is likely to be a professional like a GP or social worker.
COP4 – Deputy Declaration
This is further information about your circumstances including your financial circumstances.
